FERPA Opt Out Notification (All Grades)
The following two forms will only be completed and returned if it is pertinent to the student:

Authorization for Medication/Treatment (Prescription or Over-the-Counter)
- Health Screenings-Parent Opt Out (Grades K, 1, 3 and 6)
